UPK Application steps:
1. Go to https://upk.colorado.gov
2. Select SUBMIT AN APPLICATION 
3. Enter Parent and Child information
4. Review UPK Eligibility Report Screen
5. Complete the questions
6. Search and Browse participating providers
7. Select SEARCH BY NAME

Parents will receive a confirmation email from support@getbridgecare.com. Please read the email to confirm the desired placement was requested as #1.

Reminder: Always check your junk/clutter/spam folder.  

UPK Colorado will send student names to schools in batches.

As schools receive student information, parents will be called to submit the following within 2 days:

●   A proof of residence (includes parent’s name and address, and dated within the past 30 days)

●   Birth certificate (new students only)

Schools review applications and notify UPK Colorado of children accepted or denied placement. 

The state will email UPK updates to the parent.

Denial and match with 2nd choice.

If offered placement, parents must accept the placement ASAP to avoid being removed from the process.

UPK Colorado will notify the school that the placement was accepted.

• ASQ-SE2 online
• Early Intervention Questionnaire with Health Assessment (EIQ)    

No forms needed

When a space is available, the parents will be notified to complete Step 5: Registration. 

New students: watch your mailbox in early June

Returning students: watch your backpack the last week of school

The letter will include Registration steps, required paperwork, an academic calendar and much more!

• Backpack large enough to hold a two-pocket folder
• Water bottle (will be sent home each day to be cleaned)
• Teachers will provide information at orientation concerning change of clothes, diapers, pull-ups, wipes.
• Teachers will notify parents as supplies are needed throughout the school year.
